5. Customizing Visits – Adding Form Sections to the Online Questionnaire

Customize visits in DentalWriter Plus+ to add form sections to the online questionnaire. At the end this article is a hands-on tutorial for you to practice.

*Important Note* It is recommended to customize in a TEST patient chart using a NEW visit. You can customize in a real patient chart, but please note if you add/remove/edit items in a form section and apply the customizations to that form, it will alter any data entered in that section. Also, please keep in mind that any customization or adding or removing form sections will affect that specific visit and any future visits for all patients. The changes are not retroactive. 

To add form sections to the online questionnaire:

1.  Start by opening a test patient chart. Choose the Visits & Forms tab and then select the blue +NEW VISIT button.

2.  Select the visit you wish to customize, then select the green CREATE button.  Note: when customizing in a test chart, if the box in the lower left corner is selected to import data from a previous visit, click the box to remove the check mark.

3.  Once the visit is open, select the ellipses (3 horizontal dots) next to the title of the visit. Then select the option Activate form sections for online questionnaire.

4.  Here you can view the form sections that are currently used in the patient questionnaire. In this example a new Berlin form section was added for the Berlin questionnaire. By default, when a new form section is created it will be activated. You can uncheck it to deactivate or leave it if you want it asked in the questionnaire. You can also see additional default sections you can add to the questionnaire. Feel free to explore some of these options on a test patient in your DentalWriter Plus+ account. Click CANCEL if no changes were made and click SAVE if they were.

5.  The next step is to customize our Patient Questionnaire. In order to do that click the Admin tab and then click on Online Questionnaire Setup.

6.  Select the type of questionnaire you are going to be customizing. For this example, select Sleep Patient Questionnaire.

7.  Select the + button to add your new form section to the Sleep Patient Questionnaire.

8.  You can use the up and down arrows to move the new form section to your desired location. Make sure to click SAVE when all changes have been made. The new form section will now be on present on the patient questionnaire.
