How to import a Patient Questionnaire into DW+

This article will go over how to import your patient and the questionnaire they filled out into DentalWriter Plus thus starting their chart.

Once your patient has filled out the questionnaire you sent them via the link they will submit it back to you. You will receive an email notification (if setup) and a notification inside of DW+. 

Here is what it will look like inside of DW+:

To import the patient into DW+ click on "Online Questionnaires" on the left menu panel to open the Online Questionnaires. Once open you will see all the patients that are ready to be imported in will be bolded and say Ready For Import. 

Click anywhere on line item of the patient you want to import in. It will open a summary page of the information the patient filled out. On this page is where you will import the patient by clicking the green "Import Questionnaire" button in the upper right. 

Once you click the green Import Questionnaire button another box will pop open, on this box you will make sure you are importing the patient in as a New Patient or you can merge the patient into an existing chart if they are already an existing patient. This example is a new patient being imported in. Once you are sure it is a new patient click the green Import button in the lower right corner of the box. 

Once you click the green Import button it will pull all the patient data into DW+ and will open into the patient's chart. You will know you have successfully imported your patient when you see the green check mark with "Successfully imported from Online Questionnaire" under the Personal Information title.
