Importing a Patient Questionnaire into DentalWriter Plus+

This article will explain how to import a patient questionnaire once filled out and completed online by the patient.

On the left side pane select Online Questionnaires.

Note: A red banner flag will indicate the number of patient questionnaire's that are ready to import into the software.

Select the questionnaire to begin the import.

You will be able to view or preview the personal summary or patient demographics provided by the patient, as well as form sections for this visit from what the patient has submitted. It can only be viewed at this point.

*Note that if this is an existing patient or a patient with the same last name, DentalWriter Plus+ will find that patient.

If it's a new patient record, all you will have to do is select IMPORT to bring it into DW+

Once imported, you will now have a chart for the patient.
