Setup E-mail address to receive completed notification of completion by patient

To add, modify, remove an e-mail address to receive completed notifications from online questionnaires can be performed by steps provided.

From the left side navigation bar, select and expand Admin. Then click on Online Questionnaire Setup

From middle window section, top of ribbon, select Settings,

Then scroll down to section E-mail Notification. A field is presented to enter, add, remove here.
